Service Coordinator, Services for the Elderly, Customer Guidance and Social Work, Home Care, Salo - Varsinais-Suomen hyvinvointialue - Salo, Finland

This role combines customer work, decision-making, and management of service entities. Your tasks You will work in the Southwest Finland Wellbeing Services County in the results group for services for the elderly, where you will be responsible for services supporting independent living at home and home care service guidance. Your duties include assessing service needs, guiding and advising clients, and managing customer relationships. Additionally, you will prepare and make official decisions and participate in gerontological social work. As a home care service coordinator, you will handle processes related to applications for home care support from start to finish. Your work includes processing applications, home visits, and guiding and advising clients. You will coordinate support services needed by clients, make support decisions, and prepare matters related to payments. You will support clients through phone conversations, meetings, and group sessions, and prepare decision processes in cases of changes. You should have resilience under pressure, organizational skills, and readiness for independent decision-making and quick problem-solving. Additionally, a good understanding of the regulated social and health care environment and skills in service need assessment and the RAI functioning capacity scale are required. A driver’s license and the ability to use your own car are necessary, along with willingness to travel within different areas with short notice. Good IT skills are essential. Previous experience as a service coordinator is considered an advantage. The qualification requirement is a relevant higher education degree in social welfare or a previous vocational qualification.
