Practical Nurse, Elderly Services, Rehabilitative Assessment Unit, Turku - Varsinais-Suomen hyvinvointialue - Turku, Finland

Your tasks You will work in the rehabilitative assessment unit for the elderly at Kurjensiipi 2, where clients are assessed and rehabilitated on a short-term basis. The unit's goal is to clarify the client's overall situation, maintain functional capacity and resources, and support independent living at home. Clients are referred to the unit through service guidance decisions or post-hospital rehabilitation needs. You will implement individual rehabilitation plans for clients as part of a multidisciplinary team. Rehabilitation is carried out in everyday activities, and you will be responsible for basic care, medication distribution, and wound care. You will make daily entries on the progress of the rehabilitation process, prepare care and service plans, perform RAI measurements, and utilize other assessment tools. Additionally, you will participate in rehabilitation meetings and work closely with the client's relatives and the entire service network. In this role, you can utilize and develop your skills in elderly care and actively participate in the clients' rehabilitation process. We require good oral and written Finnish language skills. Skills in Swedish, a valid LOVe medication license, experience in geriatric and rehabilitative nursing, RAI expertise, and good IT skills are considered advantages. The qualification requirement is a professional degree in social and health care (practical nurse) or an equivalent previous qualification, along with a valid registration in JulkiTerhikki and JulkiSuosikki registers.
