Home rehabilitation operates in close collaboration with hospitals in the Western region, Seniori-info, and Lohja home care, responsible for the care and rehabilitation of elderly patients discharged from hospital at their homes, as well as assessing service needs together with Seniori-info. The home rehabilitation team includes nurses and therapists. We also work closely with other social and healthcare professionals to serve the best interests of the client. The goal is to enable clients to maintain an active daily life and support independent living at home. The work of a nurse in home rehabilitation is diverse and independent. Good communication skills, responsibility, and teamwork are emphasized. We expect that customer orientation is important to you. In addition to your team, your work is supported by the home care supervisor, therapists, and the home care doctor. The nurse's work in home rehabilitation involves 2-shift work on all days of the week. You will have the opportunity to influence your own shifts. The role requires vaccination protection according to the Communicable Diseases Act §48 and tuberculosis screening according to §55. A driving license is essential for the role. There is an option to use a work car. Kilometer allowances are paid for the use of your own car. The selected person must present a criminal record extract related to working with elderly persons, as required by the law on the supervision of social and health care (Act 741/2023, Section 28), if the employment or combined employments last over three months within a year.
