In the rehabilitative assessment team, you will help build a safe return home and support clients' independence according to their individual needs. Your tasks You will work in the rehabilitative assessment team and be involved when clients are discharged from hospital and support them after discharge. Clients also include individuals already at home who are not in the service system but are referred through the assessment unit for rehabilitative assessment before possibly transitioning to home-based services. You will conduct client visits independently. Client care and assessment are carried out multidisciplinary, with time allocated based on client needs. The duration of the assessment period varies according to individual rehabilitation needs. Your work includes collaborating with service guidance to arrange necessary services at home to support clients' independence. As a nurse, you are responsible for daily client care and its assessment, documentation, and management of related measurement tools. You will guide and advise on health promotion and participate in planning and implementing the rehabilitation process. The work emphasizes independent judgment and decision-making skills, as well as familiarity with various elderly care partners. You will act as an expert in your field within multidisciplinary teams and maintain care networks. The work is varied and performed in two shifts every day of the week. This is the current practice, which may change in the future, possibly moving nurses to a single shift. Due to the nature of the work, a valid driver’s license is required, and the use of a personal car is considered an advantage. What do we offer? In addition to the individual salary, a recruitment bonus of 8% (249.28 €/month) is available for six months from the start of employment. What do we expect from you? We expect you to have an independent work approach, good communication and documentation skills, and an interest in rehabilitative and assessment nursing at clients' homes. We value a proactive attitude towards development and a desire to help build new operations. It is an advantage to have a valid LOVe medication license, experience working with the elderly, and OMNI RAI competence. The working languages are Finnish and Swedish, and the job requires good oral and written skills in either Finnish or Swedish, with satisfactory skills in the other language. The qualification requirement is a license as a registered nurse under the Health Care Professionals Act (559/1994) and registration with the National Supervisory Authority for Welfare and Health.
