We are seeking a registered nurse for a fixed-term position in the Discharge Support Team in the northern area of Järvi-Pohjanmaa (Alajärvi, Vimpeli, Lappajärvi). The purpose of the Discharge Support Team is to support the client's functional capacity and life management so that living at home is possible for as long as possible. Clients include those discharged from wards or those already living at home who face challenges in managing at home. Most clients are elderly. The care period in the discharge support team varies from a few days to several weeks. In this role, you will work as part of a multidisciplinary team in collaboration with relatives. The position requires strong professional skills and a desire to develop professionally, the ability to adapt to changing situations, responsibility, reliability, and knowledge of rehabilitative methods. We expect good cooperation and communication skills, flexibility, and a proactive attitude towards development. Additionally, we require a motivated and conscientious approach to work, an independent work style, and a desire to provide high-quality, customer-oriented care. Previous experience working with elderly people and in home care is an advantage. The work is mobile and requires the use of a personal car. It is a two-shift work. The qualification requirement is a degree as a licensed nurse (Act 559/1994). The selected candidate must have completed a medication permit or obtain one immediately upon starting employment. Terms of employment and salary are determined according to the SOTE agreement, with a starting salary of €3050.40/month. A trial period applies at the start of employment. A criminal record extract according to law 741/2023 §28 must be presented before employment begins. The candidate may be required to undergo a drug test. HYVAEP is a smoke-free workplace. The position requires vaccination protection according to the Infectious Diseases Act §48 and a certificate of tuberculosis screening as per §55.
