Home Care Assistant, Evaluation Team - Vantaan ja Keravan hyvinvointialue - Remote - Global

We are seeking two full-time home care assistants for our evaluation team in Vantaa. The role involves supporting clients' daily activities, medication management, care procedures, ongoing assessment, service needs and RAI CHA evaluations, and mapping support service needs. The work is mobile, with home visits across Vantaa and Kerava using company cars. The team includes 16 home care assistants, 4 nurses, a client manager nurse, 7 service coordinators, and a supervisor. Clients are mainly elderly discharged from hospital needing temporary home visits to support safe return home. The goal is to enable clients to live at home despite health or functional changes and support early hospital discharge. The work location is well-connected in Tikkurila, Vantaa. The position starts on 03.08.2026 or as agreed, with a 2-shift schedule, and requires a valid driver's license and good driving skills. The monthly salary is 2739.72 EUR plus experience-based supplements. The employment is permanent, full-time, and based on the Social and Health Care Collective Agreement.
