Job Description
As a memory coordinator, you will support patients with memory disorders and their relatives throughout the care pathway. You are responsible for monitoring individuals with memory impairments and ensuring the accuracy of treatment and rehabilitation plans. You act as a link between the patient, relatives, and various professionals to ensure smooth and timely care. Your tasks include: - Assessing and monitoring the well-being, care, and overall treatment of individuals with memory impairments - Conducting cognitive tests (e.g., CERAD) and interpreting results - Evaluating the patient's functional capacity, daily living skills, and support needs - Planning and monitoring treatment and rehabilitation programs in collaboration with doctors, memory nurses, home care, social services, rehabilitation, and other services - Serving as secretary and organizer for the rehabilitation working group - Conducting home visits to assess the situation in the patient's own environment. You will have the opportunity to utilize and deepen your knowledge of memory disorders, memory testing, and functional capacity assessment. You will work closely with doctors, memory nurses, home care, social services, rehabilitation, and other stakeholders, becoming part of a broad multidisciplinary network. At the same time, you will have the opportunity to work independently and influence your own work as a coordinator of treatment and rehabilitation processes. The role requires experience working with memory disorders and proficiency in Finnish and Swedish. Experience in conducting memory tests (e.g., CERAD, Moca, and other functional capacity tests) and working in a clinic setting is an advantage. We expect you to have the ability to work independently, good communication and teamwork skills, and a desire to work for the benefit of individuals with memory impairments and their relatives. Experience working with older adults and memory issues or willingness and commitment to training in these areas are appreciated. The role requires a driver’s license and access to a personal car, as home visits are part of the job. Memory nurse training is an advantage. The qualification requirement is a suitable higher education degree or a relevant previous vocational qualification (e.g., nurse or public health nurse).
