Licensed Practical Nurse, Discharge Support Team (Kauhava-Evijärvi) - Etelä-Pohjanmaan hyvinvointialue - Kauhava, Finland

We are looking for a licensed practical nurse for a permanent position in the Discharge Support Team in Northern South Ostrobothnia, Kauhava-Evijärvi team. The purpose of the Discharge Support Team is to support the client's functional capacity and life management so that living at home is possible for as long as possible. Clients consist of those discharged from wards or those already living at home who face challenges in managing at home. Most clients are elderly. The care period in the discharge support team varies from a few days to several weeks. In the team, you work as part of a multidisciplinary team in collaboration with relatives. The role requires strong professional skills and a desire to develop professionally, the ability to adapt to changing situations, responsibility, reliability, and knowledge of rehabilitative methods. We expect good cooperation and communication skills, flexibility, and a proactive attitude towards development. Additionally, we require a motivated and conscientious approach to work, an independent work style, and a desire to provide high-quality, customer-oriented care. Previous experience working with the elderly, home care, and sample collection skills are considered an advantage. The work is mobile and requires the use of a personal car. The work is on a two-shift basis. The qualification requirement is a basic degree in social and health care, a licensed practical nurse (qualification according to Act 559/1994 and 817/2015) or an equivalent previous degree. Registration with the National Supervisory Authority for Welfare and Health (Valvira) as a protected title professional is required. The selected candidate must have completed a medicine license or do so immediately at the start of employment. Terms of employment and salary are determined according to the applicable SOTE agreement, with a task-specific salary of 2706 euros/month at the start of employment. A probationary period applies to the employment. A certificate of health status and a criminal background check according to law (741/2023) § 28 must be presented before starting the position. The selected candidate may be required to undergo a drug test. HYVAEP is a smoke-free workplace. The position requires vaccination protection according to the Infectious Diseases Act § 48 and a statement regarding tuberculosis of the respiratory tract § 55.
