Home Care Assistant, Evaluation Team - Vantaan ja Keravan hyvinvointialue - Remote - Global

We are looking for two full-time Home Care Assistants to join our Evaluation Team in Vantaa. The role involves supporting clients with daily activities, medication management, performing care procedures, ongoing assessment of overall situation, conducting RAI CHA assessments, and identifying support service needs. The work is mobile, with home visits across the Vantaa and Kerava regions using company cars. Candidates should have good ICT skills, experience with Apotti and RAI assessments, a valid medication management license, a driving license, and the ability to adapt to changing situations. The team includes 16 home care assistants, 4 nurses, a client coordinator nurse, 7 service coordinators, and a supervisor. The clients are mainly elderly individuals discharged from hospital needing temporary home visits to support safe discharge and enable living at home. The position starts on August 3, 2026, or as agreed, with a permanent contract. The salary is €2,739.72/month plus experience-based bonuses.
