Practical Nurse, Elderly Services, Rehabilitative Assessment Unit, Turku - Varsinais-Suomen hyvinvointialue - Turku, Finland

Your tasks You will work at Kurjensiipi 1, where assessments and short-term rehabilitation are conducted. The unit's goal is to clarify the client's overall situation, maintain functional capacity and resources, and support independent living at home. Clients are directed to the units through service guidance decisions or post-hospital rehabilitation needs. Your responsibilities include implementing individual rehabilitation plans for clients as part of a multidisciplinary team. You will carry out rehabilitation activities in daily routines, be responsible for basic care, medication distribution, and wound care, and make daily entries on the progress of the rehabilitation process. You will prepare care and service plans, perform RAIm assessments, and utilize other evaluation tools. Additionally, you will participate in rehabilitation meetings and work closely with clients' relatives and the entire service network. The work is on a three-shift schedule. What we expect from you We require good spoken and written Finnish language skills, experience in elderly care and rehabilitation, and strong communication and teamwork skills. You are responsible and independent in your work, familiar with the RAI assessment tools, and have good IT skills. An active LOVe medication license and Swedish language skills are considered an advantage. The qualification requirement for the position is a professional degree in social and health care (practical nurse) or an equivalent previous education in the field, and current registration in the JulkiTerhikki and JulkiSuosikki registers.
