Practical Nurse, Elderly Services, Rehabilitative Assessment Period at Home, Kaarina - Varsinais-Suomen hyvinvointialue - Kaarina, Finland

In your role, you will combine assessment, rehabilitative work, and strengthening the client's independent living at home. You will work during a rehabilitative assessment period in the client's home environment for a scheduled, goal-oriented, and time-limited period. The period begins with an assessment of service needs conducted by a service coordinator or planned hospital discharge, but concerns from the client or their relatives about managing at home can also initiate the process. You will implement enhanced and intensive home care, focusing on supporting the client's functional capacity and independence through rehabilitative measures. You will work in a multidisciplinary team and participate in an evaluation with the client and their relatives at the end of the period to determine the need for continued services. The work is independent, diverse, and variable. A driver's license and personal vehicle are essential. The work is performed in two shifts on weekdays and weekends. What we expect from you - Experience in elderly services and a rehabilitative approach - Ability to work independently and systematically in the client's home - Familiarity with RAI assessment tools or willingness to learn their use - Enthusiasm for learning new skills and flexibility in changing situations - Good spoken and written Finnish language skills. Eligibility requirements include a relevant social and health care qualification or equivalent education, or previous formal education, and registration with the National Supervisory Authority for Welfare and Health.
