Licensed Practical Nurse, Elderly Services, Rehabilitative Assessment Period in Unit, Kaarina - Varsinais-Suomen hyvinvointialue - Kaarina, Finland

You will work in the rehabilitative assessment unit Visiitti as part of a multidisciplinary team. Clients are referred to the unit through a service guidance decision or after a hospital stay due to the need for continued rehabilitation, and you will work closely with the client's entire service network. You are responsible for implementing nursing care and services that promote the client's rehabilitation, and you participate in planning, implementing, and evaluating nursing work and health promotion as part of a multidisciplinary team. You utilize tools such as RAI to assess the client's functional capacity and plan care. You monitor the client's health status and rehabilitation progress, report changes, and document nursing work in compliance with data protection practices. Additionally, you will guide and advise clients and their relatives, and participate in orientation of new employees, resource pool workers, and students. The goal of the work is to support clients in living at home. The practical nurse works in a 3-shift schedule. The qualification requirement is a basic degree in social and health care (practical nurse) or an earlier school-level degree in the field, and a valid registration in JulkiTerhikki and JulkiSuosikki registers. The role involves working in Kaarina, Finland, with a full-time schedule, in a 3-shift pattern, and requires Finnish language skills. The salary starts from €2654.89/month.
