Your work directly influences clients' ability to continue living in their own environment with as much independence as possible. Your tasks You will work in a multidisciplinary team consisting of a doctor, responsible nurse, nurse, practical nurse, social counselor, physiotherapist, and occupational therapist. The work is diverse, varied, evidence-based, and carried out in close cooperation with different professional groups. You will be responsible for planning, implementing, and evaluating nursing care and services that promote client rehabilitation. You will ensure the implementation of the client's individual care and rehabilitation plan, as well as the planning, implementation, and evaluation of medication management in the unit together with the responsible nurse. The role emphasizes nursing expertise, monitoring the client's overall well-being and rehabilitation, and assessing functional capacity using available tools such as RAI. Additionally, you will participate in onboarding and guiding new employees, resource pool staff, and students. The work involves two-shift rotation. What we expect from you We require comprehensive medication management skills and knowledge of the medication process, the ability to manage multiple tasks, and readiness to assess and monitor the client's overall condition as part of a multidisciplinary team. We value your skills in functional capacity assessment, care planning, and willingness to share your expertise by mentoring and guiding others. Good Finnish language skills are required. It is an advantage to have a valid LOVe medication license, familiarity with the RAI assessment system, and good IT skills. The eligibility requirement for the position is a license as a registered nurse according to the Health Care Professionals Act (559/1994) and registration with the National Supervisory Authority for Welfare and Health.
