Licensed Practical Nurse, Discharge Support Team, Järvi-Pohjanmaa - Etelä-Pohjanmaan hyvinvointialue - Alajärvi, Finland

We are looking for licensed practical nurses (2 positions) for permanent employment in the Discharge Support Team in Järvi-Pohjanmaa. The purpose of the Discharge Support Team is to support the client's functional capacity and life management so that living at home is possible for as long as possible. Clients consist of those discharged from wards or those already living at home who face challenges in managing at home. Most clients are elderly. The care period in the discharge support team varies from a few days to several weeks. In the team, you work as part of a multidisciplinary team in collaboration with relatives. The role requires strong professional skills and a desire to develop professionally, the ability to adapt to changing situations, responsibility, reliability, and knowledge of rehabilitative methods. We expect good cooperation and communication skills, flexibility, and a proactive attitude towards development. Additionally, we require a motivated and conscientious approach to work, an independent work style, and a desire to provide high-quality, customer-oriented care. Previous experience working with the elderly, home care, and sample collection is considered an advantage. The work is mobile and requires the use of a personal car. It is a two-shift work. The qualification requirement is a basic degree in social and health care, licensed practical nurse (in accordance with Law 559/1994 and 817/2015) or an equivalent previous qualification. Registration with the National Supervisory Authority for Welfare and Health (Valvira) as a licensed professional is required. The selected candidate must have completed a medication permit or will do so immediately at the start of employment. Employment terms and salary are determined according to the SOTE agreement, with a task-specific salary of 2706 euros/month at the start of employment. A trial period applies to the employment. A criminal record extract according to law (741/2023) §28 must be presented before starting employment. The candidate may be required to undergo a drug test. HYVAEP is a smoke-free workplace. The position requires vaccination protection according to the Infectious Diseases Act §48 and a certificate of tuberculosis screening according to §55.
