About the role

The Satakunta wellbeing services county is seeking a Service Coordinator for the informal care unit within the home care and home-based support services sector for the elderly in the northern region. The primary place of work is Pori, but travel throughout the wellbeing services county is required as needed. The informal care unit processes applications for informal care support for clients of all ages. The Service Coordinator provides guidance and counseling on informal care support, as well as conducting service need assessments and making service decisions. Based on the assessment of service needs, the Service Coordinator makes positive or negative official decisions regarding informal care support. RAI assessment is part of the service need assessment. Support for decision-making is available from the team when necessary. During service need assessments, the client's need for social and health services and the overall situation are mapped through home visits. The need for care and supervision is determined individually in all cases. The Service Coordinator also arranges other services required by clients, such as home care and day activities. We offer sports, culture, and wellbeing benefits, as well as free gym access in Rauma, Harjavalta, Pori, and Ulvila. We provide occupational health care and a comprehensive range of internal training. Eligibility requirements for the Service Coordinator position are a university degree in social services or a previous college-level degree in social services (e.g., Socionomi, Geronomi, Rehabilitation Coordinator) and qualification in accordance with Section 8 of the Act on the Qualification Requirements for Professional Social Care Personnel (817/2015). Qualifications will be verified via Julkiterhikki/Suosikki/Valvira. Good oral and written proficiency in Finnish is required. Additionally, work experience in the social sector and good ICT skills are considered an advantage. The role involves many home visits. A driver's license and the possibility of using a private car are required. Good interaction skills, an independent work style, versatility, and experience in service coordination will assist in the performance of duties. The selected candidate must present a criminal record extract as required by Act 741/2023 (effective 1.1.2024) before assuming the position, no later than 30 days after notification of the selection decision. The position is subject to a 6-month probationary period. The terms of employment are determined by the SOTE agreement. The salary is determined by the SOTE agreement, and currently, the basic salary for the position is €3,146.75/month. Working hours are standard working hours, with the possibility of flexible meal breaks. Partial remote work is possible by agreement. Interviews are planned to be held in week 35.
