About the role

Hello memory care specialist! Are you interested in independent and responsible work where you can participate in developing operations and develop yourself? Come be part of the Geriatric Outpatient Clinic's multidisciplinary community, where work is done with a big heart! The goal of the Memory Coordinator's work is to support the memory client's home living and be a support for their family. The work includes a broad mapping of the resources and overall situation of a person with memory symptoms living at home, as well as the planning, coordination, and monitoring of the client's care, rehabilitation, and services in a multidisciplinary network. The work is independent daytime work on weekdays and is multi-site in nature, at the homes of clients and relatives as well as in the office. Eligibility requirement: Bachelor of Nursing degree or previous institute-level degree or specialist nursing degree or public health nurse or midwife degree, which includes legalization as a registered nurse. The position requires legalization in Finland according to the Act on Health Care Professionals (559/1994, 1200/2007). Persons with a Bachelor of Social Services degree or a corresponding previous institute-level degree are also suitable for the position. Language requirements: good oral and written skills in Finnish and satisfactory skills in Swedish. The position requires vaccination protection according to Section 48 of the Communicable Diseases Act and a tuberculosis certificate according to Section 55. The position requires the presentation of a criminal record extract for those working with elderly people (Act on the Supervision of Social and Health Care 741/2023, Section 28). The person selected for the position must present a certificate regarding a drug test to the employer before the employment begins. Before the selection decision, the applicant must present an occupational health assessment of their ability to perform the tasks included in the work. The position requires the successful completion of the LOP/LOVe LOP medication permit by the end of the probationary period at the latest. Additionally, we require: The position requires good interaction and cooperation skills, ability for team and network work, as well as overall situation assessment and independent decision-making. We consider as an advantage: - work experience with people with memory diseases - special studies in memory work - user experience with the Apotti patient information system - good IT skills - good knowledge of the City of Helsinki's social and health services NOTE! Interviews will be held on Tuesday 8/25, Friday 8/28, and Monday 8/31.
