We are seeking a Public Health Nurse to join the multidisciplinary Employment Services network (TYM) in Kerava. The Nurse will conduct health screenings for clients and assess factors related to work ability in relation to the client’s multidisciplinary employment plan. The role includes motivating and guiding clients to maintain health and work capacity and directing them to necessary health care services. The nurse works as part of a multidisciplinary team and collaborates with various actors to find a suitable service package for the client. In addition to health screenings for the unemployed, the TYM nurse conducts multidisciplinary initial assessments under the TYM act and participates in updating legally mandated multidisciplinary employment plans with the employment expert, social counselor and Kela expert. All TYM nurses participate as experts in VAKE’s multidisciplinary work capacity team on a rotating basis. The position is new to Kerava TYM services and involves substantial development work in collaboration with Kerava’s employment services TYM team. You will have strong frontline leadership support from the Vantaa TYM services and experienced colleagues. We are looking for a customer-oriented service professional with motivation and the ability to work independently with clear goals. Networking comes naturally to you and your collaboration and communication skills are strong. You can take on large responsibilities and apply your expertise to practical nursing duties.

- Conduct client health screenings and assess work ability factors in relation to multidisciplinary employment plans
- Motivate and guide clients in maintaining health and work capacity
- Provide guidance to access necessary health care services
- Collaborate with multidisciplinary team and external partners to tailor service packages
- Participate in multidisciplinary initial assessments and updating employment plans per SOTE agreements
- Engage in VAKE work capacity team on a rotating basis
- Support development of new operating model with Kerava TYM team
- Participate in frontline leadership and continuous improvement
Requirements
- Experience in health screenings (esp. for unemployed)
- Experience in TYM work is a plus
- Understanding of welfare services and rehabilitation
- TYPPI system knowledge
- APOTI system knowledge
- Willingness to work in a multidisciplinary team
