There is now an opportunity to work in a broader-than-usual job description in a combined role of home rehabilitation and safety assistance! We are seeking practical nurses for permanent positions in Home Rehabilitation in Raasepori and Lohja. In addition to home rehabilitation, the role includes shifts in safety assistance tasks. The working area may change or expand as needed. Home Rehabilitation works in close cooperation with the safety assistance service, the Western hospital area, Senior-info, and regional home care, handling safety assistance visits, the care and rehabilitation of elderly people discharged from the hospital at home, and service need assessments together with Senior-info. The work of a practical nurse in home rehabilitation is independent, client-oriented, and rehabilitative nursing care taking place in the client's home, including medical tasks in addition to basic care. The goal is to enable the client's functional daily life and support home living. Our clients are users of home rehabilitation and safety assistance services in all municipalities of the wellbeing services county. Most clients are elderly. In the safety assistance shifts included in the practical nurse's work, your key responsibilities are receiving alarm calls from safety assistance clients via the electronic alarm center system and deploying to the homes of various clients alone and independently. You perform a care need assessment and situational assessment for the client and provide the necessary immediate medical first aid or other support. You assess the need for additional help and arrange follow-up care for the client if necessary. If needed, you consult other professionals and ensure that the client receives sufficient help at home. Additionally, you are responsible for making necessary entries into the system and reporting to other professionals. You collaborate with various authorities, such as home care and social and crisis emergency services. Your work also includes installing safety phones and other safety devices for clients and checking the functionality of safety devices. In this job, you will certainly be able to utilize the diverse skills of a practical nurse! Good interaction skills, responsibility, and teamwork skills are emphasized in the work. Supervisors and the work unit are there to support you and ensure your work proceeds smoothly. You are part of a multidisciplinary team which, in our unit, includes practical nurses, registered nurses, physiotherapists, occupational therapists, and a doctor. The work is shift work, including morning and evening shifts as well as weekend work. Suitable education: The eligibility requirement for the position is a degree of practical nurse, with a specialization in basic level emergency care or nursing and care. A rescue worker's degree with a basic level emergency care specialization is considered an advantage. From 1.1.2024, the person selected for the position must present a criminal record extract necessary for verifying the criminal background of a person working with elderly people in accordance with Section 28 of Act 741/2023.
