Your Role A rehabilitative assessment period is initiated based on a service need assessment, a planned discharge from a hospital, or concerns raised by the client or their relatives. You will implement enhanced and intensive home care, focusing on supporting the client's functional capacity and independent performance through rehabilitation-promoting activities. During the period, you will work closely with a multidisciplinary team and participate in the final assessment, where individual services to support and secure home living are defined together with the client and their relatives. The work is independent, versatile, and varied, and you will receive strong support from the team and supervisor. What We Expect From You We expect an interest in rehabilitative home care for the elderly, the ability to work independently, and a desire to act as part of a multidisciplinary team. We value enthusiasm for learning new things and flexibility in changing situations. The position requires good Finnish language skills, a driver's license, and access to a private car. Work experience in elderly services, familiarity with the RAI assessment tool, and a valid LOVe medication permit are considered an advantage. The eligibility requirement for the position is a vocational qualification in social and health care (Practical Nurse) or an earlier school-level degree in the field, as well as valid registration in the JulkiTerhikki and JulkiSuosikki registers.
