We are seeking a substitute service coordinator for the period 24.8.–31.12.2026 for the general medicine wards of the Wellbeing Services County of Southwest Finland. The primary area of operation is Pargas and Kimitoön, but you will work in close cooperation with the service network of the entire wellbeing services county. As a service coordinator, you will play a key role in ensuring that clients' follow-up care and necessary services are arranged smoothly and in a timely manner. You will work in multidisciplinary cooperation with ward staff, social and health care services, relatives, and other partners. Your work combines a client-oriented approach, solution-centricity, and the management of broad overall processes. Your days will be varied, and you will have a concrete impact on the coordination of client services and the smoothness of care pathways. Working hours are office hours (38:15). The start date for the substitution is negotiable. The position requires good command of Finnish and Swedish. Swedish language skills are considered a significant advantage in this bilingual operational area. Eligibility requirements include a suitable university degree or a suitable previous vocational college degree. A Bachelor of Social Services (AMK) is considered an advantage. Interested?
