About the role

We are seeking a ward pharmacist for a permanent position at Päijät-Hämeen keskussairaala in Lahti. The hospital pharmacy operating within the Päijät-Häme region provides medicines logistics services to the health care district. About 55 professionals work in the hospital pharmacy, with nearly 30 on wards. In addition to drug deliveries, our focus areas include drug preparation, preparing cytostatic agents, and ward pharmacy. You will join a multidisciplinary team and work on the surgical ward R3 (orthopedics and traumatology; 25–30 beds). The role will be evolved into a clinical pharmacy specialist position in collaboration with other professionals. Tasks include updating patients’ medication lists in the patient information system, performing medication reviews, and supporting smooth ward care and safe transitions to further care or home. You will educate patients on correct medication use and address medication-related challenges in collaboration with patients and ward staff. This work aims to promote safe, effective, and impactful pharmacotherapy. Developing medication-use processes is a central part of the role. You will also be responsible for preparing IV medications, receiving pharmacotherapy evidence, training care staff, and delivering various medication information. On surgical wards R4 and R5 two other ward pharmacists work alongside you, providing collegial support. There is also a ward medication worker in the logistics of medicines, and new technologies such as smart medication cabinets have been adopted.
