Kotikuntoutus collaborates closely with Espoo Hospital, Seniori-info and Espoo home care to address the care and rehabilitation needs of older patients discharged from hospital. The home-based practical nurse role involves independent, patient-centred and rehabilitative nursing at the patient’s home, in addition to basic care and nursing duties. The aim is to enable functional daily life and support aging in place. As a practical nurse you will work as part of a multidisciplinary team that includes other practical nurses, nurses, physiotherapists, occupational therapists and a physician. The work utilizes a broad range of nursing skills. It is a team-based role that emphasizes good communication, responsibility and teamwork. Supervisors and other unit staff support you to ensure smooth work. The position is shift-based, including morning and evening shifts and weekends. The position is temporary from 7 Sept 2026 (or as agreed) to 5 Sept 2027, with possibility for permanent employment later. Qualifications: For a practical nurse, the title must be a protected professional title. Proficiency in Finnish (spoken and written) is required. A driving license is mandatory. A medicine administration license is not mandatory during recruitment but must be completed during the probationary period. Vaccination protection under the Infectious Diseases Act and tuberculin screening under the same act are required. From 1 Jan 2024, a police background check in accordance with the Act 741/2023 §28 is required for anyone working with older persons.
