The role combines independent decision-making, a rehabilitative approach, and multidisciplinary collaboration to promote client wellbeing. In the rehabilitation assessment team you are closely involved as a client is discharged from hospital and you support them after returning home. The client may also be someone living at home who is not yet in the service system but referred to the rehabilitation assessment period by the service needs unit before possible transition to home-based services. You work independently in clients' homes but as part of a multidisciplinary team. The care plan and assessment are designed together with different professional groups, allocating time for client work according to individual needs. The assessment period length varies according to the client’s rehabilitation needs. Your duties include daily nursing care, care assessment, documentation, and management of assessment tools. You guide and advise clients in health promotion and participate in planning and implementing the rehabilitation process. You work closely with service guidance to arrange the services a client needs to cope at home. You function as an expert in your field within multidisciplinary workgroups and maintain collaboration networks with various elder-care actors. The job emphasizes independent judgement, decision-making, and familiarity with elder-care service networks. The work is varied, and nurses work morning shifts on weekdays.
