In this role you will provide meaningful care in clients' own homes and contribute to the development of communal living as part of a small, efficient team. Your duties include supporting elderly wellbeing, health, functioning and participation in their living environment, delivering rehabilitative care, and participating in planning, execution, and evaluation of care within a multidisciplinary team. You will handle daily care tasks (basic nursing, medication administration, SKE activities) and assist with maintaining client independence. You will also be responsible for documentation and on-call leadership during shifts. Close collaboration with families and other stakeholders is essential, as is involvement in unit development, student supervision, and onboarding of new staff. The role also includes supportive tasks in food and dishwashing. Work is two-shift with shared scheduling, offering you the chance to influence your own shifts.
