Lähihoitajana kotihoidossa työskentelee itsenäisesti mutta tiimin tukemana. Työtehtäviin kuuluu asiakaslähtöisen hoidon suunnittelu, itsenäinen toteutus sekä arviointi. Tiimi on moniammatillinen ja yhteisöllinen; tehdään tiiviissä yhteistyössä muiden alueiden kotihoidon sekä kotiutumisen ja arvioinnin tuen tiimien kanssa. Työ on vuorotyötä sekä aamulla että iltavuorossa ja viikonloppuisin. Tarjoamme kattavan perehdytyksen ja mahdollisuuden kehittää kotihoidon toimintaa; käytämme teknologiaa osana hoitoa. Kelpoisuus sosiaali- ja terveysalan perustutkinto (lähihoitaja) tai alan aikaisempi ammatillinen tutkinto. Lääkeluvat, ajokortti ja oman auton käyttömahdollisuus toivotaan. Eduksi katsomme kokemusta kotihoidosta ja RAI-mittarin tuntemisen. Koeaika. Palkka 2935,15 €/kk+ lisät. Hakuaika 3.9.2026 klo 14:00. Keski-Uudenmaan hyvinvointialue vastaa alueellaan.

- Plan, implement and evaluate client-centered care in home care
- Work independently with team support
- Collaborate with multidisciplinary teams and other regions' home care and discharge/support teams
- Flexible scheduling and client-centered shift planning
