Practical Nurse

We are recruiting a practical nurse for a permanent position in the northern area of the Discharge Support Team (Kauhava–Evijärvi). The Discharge Support Team’s purpose is to support the patient’s functional capacity and life management so that living at home is possible for as long as possible. The patients consist of individuals discharged from wards or those already at home who have difficulties managing at home. Most of the patients are older adults. The patient’s care episode in the discharge support team lasts from a few days to several weeks. In the team you work as part of a multidisciplinary unit in cooperation with relatives. The job requires strong professional skills, willingness to develop professionally, ability to adapt to changing situations, responsibility and reliability, and expertise in rehabilitation methods. We expect you to have good cooperation and communication skills, flexibility and development-mindedness. In addition we require a motivated and conscientious attitude to work, an independent working style and the will to provide high-quality, customer-oriented care. Previous experience working with older people, home care and sampling is considered an advantage. The work is mobile and requires the use of your own car. It is a two-shift job. The eligibility criteria include a social and health care qualifications at a basic level, practical nurse (as defined by the law 559/1994 and 817/2015) or an earlier equivalent qualification. We require registration in the central register of social and health care professionals by Valvira as a protected title professional. The selected person must have completed a medical license or will complete it at the start of employment. The terms and wages are determined in accordance with the SOTE collective agreement, the task-specific pay at the start of employment is 2767.43 €/month. A probationary period applies to filling the position. A criminal background check under the Act 741/2023 §28 must be presented before the start of employment. The selected person may be required to undergo a drug test. HYVAEP is a nicotine-free workplace. The job requires vaccination protection according to the Infectious Diseases Act §48 and a certificate of tuberculosis according to §55. The location is Etelä-Pohjanmaan hyvinvointialue.
