About the role

We are seeking a new practical nurse for a fixed-term contract from August 3, 2026, to March 30, 2027, for the Oma Häme elderly home care assessment and rehabilitation team. You may also start by agreement. The duties are located in the Hämeenlinna region team, whose operational area covers the city center of Hämeenlinna, Rengo, Kalvola, Janakkala, and Hattula. Our office is located in Hämeenlinna. The Home Care Assessment and Rehabilitation Team implements assessment and rehabilitation periods for clients who are estimated to need regular home care services. The primary task is to investigate more closely, together with the client, their relatives, and other collaborating parties, the client's resources, functional capacity, and rehabilitation possibilities, as well as to actively assess the need for services. Assessment periods are carried out at the client's home, and every visit is based on a rehabilitative approach. The duration, content, and methods of the assessment and rehabilitation period are defined with the client according to their individual needs. During the period, the possibilities of utilizing technologies (medicine dispensers, remote care) in the client's care are also mapped. The goal is for the client's functional capacity to remain the same or improve during the period. As functional capacity improves, the need for home care services may decrease, or functional capacity may improve to the point where home care services are no longer needed after the assessment and rehabilitation period ends. As a member of the assessment, rehabilitation, and discharge team, you will get to develop the team's operations in a constantly changing operating environment and deepen your own professional skills in both assessment and rehabilitation. As a practical nurse, you are responsible for the holistic assessment and support of clients' ability to manage at home. You will participate in the planning and implementation of the client's service within the team in cooperation with other team members. In addition to client visits, the work includes written tasks such as performing Rai assessments and updating the client's care plan and weekly schedule. The work is independent, but you operate as part of a multidisciplinary team, working in close cooperation with the client, their relatives, and the rest of the team. Eligibility requires a vocational qualification in social and health care as a practical nurse or another equivalent degree. We require registration in the central register of social and health care professionals. Participation in medication administration requires a valid medication dispensing permit, which you can complete or update with us at the start of employment if necessary. The work is two-shift work from Monday to Sunday. The nature of the task requires the possibility of using your own car. Because our team's work is hectic and situations are changing, excellent teamwork skills are a necessity. Success in the position requires, among other things: - ability to adapt to changing situations - good stress tolerance - excellent interaction skills - desire to develop and improve the team's operations - expertise in medication administration Additionally, experience in home care and elderly care, as well as a positive attitude towards rehabilitation and rehabilitative nursing care, are considered an advantage. We arrange interviews during the application period and may proceed with recruitment once a suitable person is found. The position-specific salary is €2,750.80/month. In addition to the position-specific salary, we pay an experience-based supplement, which can be 0%-8%. The accumulation of vacation days is also determined based on length of service. Working time is 114:45 hours in three-week periods. Applications must arrive by the end of the application period and must be submitted through the Kuntarekry system. Those working with the elderly and disabled must present a criminal record extract as referred to in Section 6.4 of the Criminal Records Act, in accordance with Section 28.1 of the Act on the Supervision of Social and Health Care (741/2023). Salary and other terms of employment are determined according to the SOTE agreement. A probationary period applies. Immediate patient and client work requires vaccination protection as required by Section 48 of the Communicable Diseases Act and a respiratory tuberculosis certificate according to Section 55.
