Practical Nurse

Practical Nurse position in the Rehabilitation Home Care unit in South Karelia Welfare Cluster. The unit provides home-based nursing services across the region, with offices in Lappeenranta and Imatra. Responsibilities include conducting functional ability and care need assessments using the ICF framework, supporting daily physical, psychological and social well-being, and performing nursing home visits. The unit is part of a multi-professional team including nurses and a physiotherapist, with physician consultation available. Emphasis is on continuous assessment of functioning and health and proficiency with information systems. Requires RAI assessment skills, independence, teamwork, strong communication, and customer-oriented approach. Work shifts are in two-person coverage with two shifts per day year-round. Qualifications: social welfare and healthcare basic degree (practical nurse/caregiver) and registration as a regulated healthcare professional. Must hold a valid prescription for medication from South Karelia Welfare Region and a Class B driving license, with access to a car. Medical certificate of fitness required within 30 days of selection; vaccination against communicable diseases is required. Infectious safety questionnaire is used for all patient work.
