The position combines client guidance, decision-making, and the coordination of housing service placements as part of seamless service paths. Your Role We are seeking a Service Coordinator for the client guidance and social work services for the elderly in the Wellbeing Services County of Southwest Finland. The role is situated within the service coordination and placement management of housing services. Tasks in client guidance and social work for the elderly include client guidance and counseling for housing services, assessment of service needs, client relationship management, and making official decisions. You will work closely with clients, their relatives, elderly service areas, other result areas, and private service providers. Client and service counseling and guidance are carried out via telephone and home visits. In the placement team, you will be responsible for coordinating housing service placements for the elderly in Varha, granting placements, decision-making, and ensuring client information is up to date. Your work supports a smooth service path, timely decision-making, and the overall housing services entity in close cooperation with the rest of the network. The work is mobile. Your primary workstation is in Kemiönsaari. What We Offer We offer a versatile and responsible position in a central operating environment for elderly services. Success in the role is supported by knowledge of the regulated operating environment of social and health care, as well as proficiency in service need assessment and the RAI functional capacity scale. The work requires a driver's license and the possibility of using your own car. Eligibility requirements include a suitable higher education degree in social services or a previous institute-level degree. Language requirements include good oral and written skills in Finnish and good oral and written skills in Swedish.
