OUR OPERATING AREA Aging care in Mariehamns city employs about 200 staff. The aging care comprises two directions: home care and housing care. The home care unit offers home services and day and drop-in activities. Nearby care includes also informal care for persons aged 65 or older. The housing care includes service housing with full-time care and institutional housing. JOB DUTIES As a service counselor you work with information, guidance and assessment in matters concerning seniors. You have a central role in supporting older people and their relatives by mapping service needs and ensuring that the right support and interventions are put in place at the right time. In the role you work independently but in close cooperation with colleagues and other actors within and outside the organization. You contribute to creating conditions for older people to live a safe and independent life for as long as possible. Your main duties include: Client work and service assessment - Provide information, guidance and coaching about services for the elderly, based on applicable legislation - Investigate and assess service needs and document client cases - Participate in care planning and coordination meetings for clients - Make official decisions regarding services and support for older care, according to delegations Collaboration and coordination - Be the contact person in client cases in cooperation with Ålands health and medical care, other municipalities and authorities - Participate in assessment groups for service needs and informal care - Handle cases such as risk reports - Coordinate measures for clients together with concerned actors Development and expert assignments - Contribute to development and follow-up of activities within the area of responsibility - Participate in collaboration meetings and act as an expert on issues concerning elder care services - Introduce and mentor new employees and students as needed QUALIFICATIONS AND COMPETENCIES We are looking for someone with a polytechnic degree as socionom (YH) or equivalent with registration in Valvira. Experience in elder care or authorities work and knowledge of legislation concerning elder care are advantageous. As a person you are flexible and you enjoy working in a structured, solution-focused way with a focus on the needs of the elderly and the ability to juggle many tasks at once. You have good collaboration skills and thrive in a role where you cooperate with various actors. You also have good computer skills and quickly learn new systems. We place great emphasis on your personal qualities. CONDITIONS The role is full-time and in employment, the working time is 38.15 hours/week with flexible working hours. Working hours are weekdays during the day. We can offer a permanent employment with a six-month probation. Start according to agreement. The base salary is €3,642.86 per month; any experience allowances are added. The assignment requires vaccination protection in accordance with §48 of the Communicable Diseases Act (FFS 1227/2016).
