As a service coordinator, you work with home-based services for older people and especially caregiver support coordination. Your designated worksite is Loimaa region (Aura, Koski Tl, Marttila, Pöytyä, Rusko). You will work in the elder care customer guidance and social work outcome group, whose duties include guiding and advising on home-based elder care services, assessing service needs, managing client relationships, making official decisions, and gerontological social work. Your tasks include handling applications for caregiver support, performing home visits, guiding and advising clients, coordinating caregiver support with other services, and making support decisions. You will prepare for support payments and assist clients in phone conversations, meetings, and group situations. You will prepare the decision process in case of appeals. Your supervisor is a service manager. We offer a responsible and varied role with independent decision-making, client contact, service coordination, and development of caregiver services. You will view the client’s situation holistically and apply your expertise to both individual services and the quality and cost-effectiveness development of caregiver services.
