Would you like your work to influence our clients' ability to live independently and safely in their own homes for as long as possible? Now there is an opportunity to work in a broader role in a joint task of home rehabilitation and safety assistance! We are hiring licensed practical nurses for permanent positions at the home rehabilitation units in Raasepori and Lohja, as well as at the Kirkkonummi and Vihd-Karkkila locations. The role includes home rehabilitation work as well as shifts in safety assistance tasks. The working area may change or expand as needed. Home rehabilitation operates in close cooperation with the safety assistance service, the Western Hospital District, Senior Info, and local home care services, handling safety visits, care and rehabilitation of elderly discharged from hospital at home, and needs assessment together with Senior Info. The work of a licensed practical nurse in home rehabilitation involves independent, client-centered, and rehabilitative care at the client's home, including basic care as well as nursing tasks. The goal is to enable clients to maintain functional daily life and support living at home. Our clients are clients of home rehabilitation and safety assistance services across all municipalities of the wellbeing area. Most clients are elderly. In safety assistance shifts, your key responsibilities include receiving emergency calls from clients via an electronic alarm system and visiting clients' homes independently and autonomously. You assess the need for care and situation, provide immediate medical first aid or other support, evaluate the need for additional help, and arrange further assistance if necessary. You may also consult other professionals and ensure the client receives adequate help at home. Additionally, you are responsible for making necessary entries into the system and reporting to other professionals. You collaborate with authorities such as home care and social and crisis emergency services. Your duties also include installing and checking alarm systems and other safety devices for clients. This role offers a comprehensive opportunity to utilize your practical nursing skills! Good communication skills, responsibility, and teamwork are emphasized. Supervisors and the work unit will support you to ensure smooth work. You will be part of a multidisciplinary team including nurses, physiotherapists, occupational therapists, and doctors. Suitable education for the role: The qualification requirement is a licensed practical nurse degree, with competence in basic emergency care or nursing and care. A basic emergency medical qualification from a rescue training is considered an advantage. Requirements: - Fluent Finnish, both spoken and written - Swedish language skills are desirable in the western area - A valid driver’s license is mandatory - Medication administration license is not mandatory but must be completed during the trial period. We hope you: - Understand the support services for living at home and client needs - Are familiar with social and health emergency and alarm center operations and relevant legislation - Are cooperative: approach people calmly and empathetically, and can quickly assess client situations and care needs - Can work independently based on the shared procedures and guidelines of the Western Uusimaa wellbeing area - Are prepared for independent fieldwork in clients’ homes. We value: - First aid skills - Experience in elderly care and current medication licenses - Ability to quickly adopt new systems - Previous experience with systems such as patient record systems, alarm systems, safety device management, and sensor data analysis platforms. The role also requires vaccination against infectious diseases as per the Infectious Diseases Act §48 and tuberculosis screening as per §55. Selected candidates may be asked to provide a drug test in accordance with the Western Uusimaa wellbeing area’s substance abuse policy. From 1.1.2024, candidates must present a criminal record extract relevant for working with elderly persons, in accordance with law 741/2023, section 28. We are agents of change.
