The Health Station Services of the Lapland Wellbeing Region are seeking a project manager for a fixed-term position to develop the continuity of care in health centers as part of the Northern Path - Omalääkäri-Omahoitaja Model Development Project, from September 1, 2026, to December 31, 2027. The position is located within the social and healthcare center responsible area, in the unit responsible for health center services. The health station services oversee the operations of the health centers in the Lapland Wellbeing Region. Key objectives of the project: The project is funded by a state grant from the Ministry of Social Affairs and Health. 1. The goal is to strengthen care continuity in the Lapland Wellbeing Region by developing a unified and practically embedded Omalääkäri-Omahoitaja model that supports personal and permanent patient relationships and considers the sector-based work structures of doctors, ensuring the provision of Sami-language services as part of the model. 2. The aim is to enhance the integration of digital services into the work of the family doctor and nurse, supporting care continuity more effectively. During the project, professionals can familiarize themselves with existing digital services and test their functionalities in their work. The goal is to lower the threshold for using digital tools and ensure they genuinely support both the patient’s care pathway and the professionals’ work manageability. 3. A central part of the project is to create a unified data-driven management practice that supports the implementation of the family doctor and nurse model and monitors care continuity. The project will define care continuity metrics (besides COCI and SLICC), standardize documentation principles, and create a clear model for data collection, monitoring, and utilization. Responsibilities of the project manager: The project manager is responsible for the overall coordination of the project, progressing according to schedule, achieving objectives, and financial monitoring. The work includes: * Managing project implementation and multidisciplinary collaboration * Planning and organizing training and communication * Supporting supervisors in change management * Monitoring and reporting on the project * Coordinating cooperation between different actors and sectors, including inter-wellbeing region collaboration. We expect from you: * Sufficient higher university degree * Qualification as a healthcare professional (554/1994) * Broad understanding of social and healthcare services, their management, and development * Experience in developing or implementing care continuity * Ability to work independently and manage projects * Knowledge of the service system in the Lapland region * Good organizational, communication, and interaction skills * Willingness to travel within the Lapland Wellbeing Region municipalities. The position requires travel within the service area. Terms of employment and salary are based on the general collective agreement for the social welfare and health care sector (HYVTES). The specific salary is €4,500/month (+ possible experience bonus). A 6-month probationary period applies. We follow the vaccination requirements as per the law and work safety regulations.
